Screens, strain and what the evidence actually supports
Long hours at a screen leave eyes sore and tired. Much of what is sold as a remedy does not hold up. Here is what the research supports.
Digital eye strain — the soreness, dryness, blurring and headache that build over a long day at a screen — is real, common, and almost entirely reversible. It is also surrounded by more marketing than nearly any other eye complaint, so it is worth separating what is established from what is merely sold.
What causes it
Three things, mostly.
You stop blinking. Normal blink rate is roughly fifteen to twenty times a minute. Concentrating on a screen cuts it by more than half, and the blinks that remain are more often incomplete. The tear film thins, the surface of the eye dries, and the result is grittiness, burning, and paradoxically watery eyes as the reflex tearing kicks in.
Your focusing system is held in one position for hours. The muscles that focus the eye and turn it inwards for near work are doing sustained effort with no break. Like any muscle held under load, they complain.
An uncorrected error is being compensated for. A small long-sighted error or astigmatism that causes no trouble at all in ordinary life becomes noticeable after eight hours of close work, because the effort of overcoming it is now continuous.
What works
Breaks, on a schedule. The widely cited 20-20-20 rule — every twenty minutes, look at something about twenty feet away for twenty seconds — is not precise science, but the principle is sound: the focusing system needs to be released periodically. Set a reminder, because you will not remember on your own.
Blink deliberately. It sounds trivial. It addresses the largest single cause. A few full, complete blinks every so often does more for screen-related dryness than most drops.
Get the geometry right. The top of the screen at or just below eye level, roughly an arm's length away, tilted slightly back. Looking slightly downward means the eyelids cover more of the eye's surface, which slows evaporation.
Fix the lighting. A bright screen in a dark room is hard work; so is a screen with a window glaring behind it. Match the screen roughly to the brightness of the room and keep windows to the side rather than in front or behind.
Have your eyes examined. If strain is persistent, the most likely finding is a small uncorrected prescription or a binocular vision problem — the eyes not converging or focusing comfortably together. Both are straightforward to identify and to correct, and neither is something you can diagnose yourself.
What does not work as advertised
Blue-light filtering lenses. These are sold hard, and the claim is that blue light from screens damages the eye or causes strain. The evidence does not support it. Systematic reviews of randomised trials have not found that blue-blocking lenses reduce eye strain compared with ordinary lenses. The amount of blue light a screen emits is a small fraction of what you get from stepping outside.
There is a separate and better-supported point about blue light and sleep: evening light exposure shifts the body clock and delays sleep onset. If that is your concern, reducing screen use in the hour before bed does more than a lens coating, and most devices have a night mode that is free.
Eye exercises sold as a cure. Structured vision therapy is a legitimate treatment for specific diagnosed binocular conditions, prescribed after an examination. Generic "eye yoga" routines promoted online as a way to eliminate your prescription are not the same thing and will not change your refractive error.
Supplements for screen fatigue. No supplement has been shown to relieve digital eye strain.
The part that is easy to miss
For children and adolescents there is a different concern, and it is not strain. Time spent on near work indoors is associated with the development and progression of short-sightedness, and time spent outdoors is protective — consistently, across a large body of research. Roughly two hours of daylight a day appears to matter. That is an argument about how children spend their afternoons, not about the screen itself.
If screen-related discomfort persists after you have fixed the blinking, the breaks and the setup, book an examination. Persistent strain usually has a findable cause.
